Quiz cannot be viewed or played	Category visibility, View/Play permissions, activity, moderation state, start/end dates
Author cannot add a question type	Quiz type compatibility, enabled definitions and author/team permissions
Player cannot choose question count	Author's optional setting, permission limits and positive section draw limits
Correct answers or explanations are missing	Separate permissions, quiz display options, unseen-question policy and Assessment behavior
Feedback is absent	Feedback permission, supported quiz type, minimum score, required answers and enabled fallback types
Defaults do not appear	A matching quiz-specific entry suppresses defaults of the same feedback type
Music does not start automatically	Click Play; check browser autoplay policy, file format, audio attachment and offset
Music import is unavailable	Music quizzes require attached audio and do not support question import
Imported media is missing	XML and GIFT do not embed attachment files
Imported questions are not public	Check processing completion and imported-question approval settings
Score differs from expectations	Question scores, selected-choice bonuses/penalties, unanswered options and music difficulty
Section breakdown is absent	Older completions have no retrospective section results
Leaderboard history is missing	History period, scheduled tasks, permissions and category/quiz eligibility
An old attempt disappears	Incomplete-attempt and guest retention settings
Layout differs from screenshots	Active style, screen width and installed version

Reproduce with a small quiz and an ordinary member account. Record the quiz type, question type, relevant settings, submitted answers and expected result. Check XenForo's server error log when a page fails to load.
